In New York, doula care is covered at no cost for Medicaid members: since March 1, 2024, New York State Medicaid has covered doula services during pregnancy and for up to 12 months after, regardless of how the pregnancy ends. Private insurance coverage varies from plan to plan, so the only reliable answer is the one your plan gives you. Below, Hudson Valley Family Doulas walks through what the New York State Department of Health says Medicaid covers and the questions to ask if you have a private plan.

Does Medicaid cover a doula in New York?

Yes. According to the New York State Department of Health, all New York State Medicaid members enrolled in fee-for-service Medicaid or Medicaid Managed Care are eligible for doula services at no cost, during pregnancy and up to 12 months after, with each pregnancy.

What does New York Medicaid cover?

  • Up to 8 doula visits before and after the pregnancy
  • Support during childbirth
  • Services up to 12 months after the pregnancy ends
  • Coverage with each pregnancy, no matter how the pregnancy ends

The care has to come from a doula enrolled with Medicaid. The Department of Health keeps a directory of Medicaid fee-for-service doulas.

Do I need a referral from my doctor?

No. New York’s State Health Commissioner issued a statewide standing order stating that all New Yorkers who are pregnant, birthing or postpartum would benefit from doula services. The order fulfills Medicaid’s requirement for a provider recommendation, so members don’t need an individual recommendation.

What if I have a Medicaid Managed Care plan?

The Department of Health notes one rule for Managed Care members. If you start doula services on or after April 1, 2025, your doula needs to participate with your Medicaid Managed Care plan for the services to be covered. If you started with a Medicaid-enrolled doula before April 1, 2025, those services stay covered until 12 months after the end of your pregnancy. Your plan can confirm which doulas participate.

Does private insurance cover a doula?

It depends on the plan. Some families pay for doula support themselves, so it’s worth calling your insurer before your due date and asking:

  • Are birth or postpartum doula services covered under my plan?
  • Does the doula need to be in network, or can I submit for reimbursement?
  • Is there a limit on the number of visits or hours?
  • What paperwork would I need to send in?
